How to use the medication log

  1. Open the worksheet and choose Print log / save as PDF. Select A4 or US Letter paper, portrait orientation and 100% scale. Turn off browser headers and footers for a clean sheet, then print or save as PDF.
  2. Use a separate sheet for each medication. Copy the name, strength, amount, unit and schedule from your veterinarian’s instructions.
  3. Record each dose after it is given, including the time and the person who gave it. Write “uncertain” when you cannot confirm an entry.
  4. Keep the sheet with the prescription instructions so everyone caring for your pet can find the same record.

These are blank organizational worksheets. They do not choose a medication, calculate a dose or tell you how to handle a missed dose. Ask your prescribing clinic about anything unclear. Do not delay contacting a veterinarian to complete a log.

Your notes stay with you

The worksheets contain no online entry fields or tracking scripts. What you write on a printed or saved copy is not submitted to PetDosageChart. This page measures worksheet-link clicks to understand which resources people use.

Share the blank worksheets
